New patient paperwork
If you’re new to our practice, you’ll need to complete intake paperwork on your first visit. Every office will have its own set of required forms, but there are some basics you’ll need to complete in almost any healthcare setting.
New patient intake forms
This basic paperwork will ask for your name, date of birth, contact information, and the party responsible for treatment. The latter is financially responsible for the treatment and will be expected to sign all consent forms, which is why they must be at least 18 years of age.
Medical history forms
The medical history form is used to evaluate your overall health before treatment. Read through each item carefully as the accuracy of this form is important to the success of your treatment plan!
Dental history forms
Similarly, a thorough dental history is essential prior to beginning orthodontic treatment. This paperwork will include an assessment of the dental work you’ve had completed over the years, as well as any history of surgery, trauma, or TMD. These forms will also ask for an outline of your current dental hygiene routine.
Dental insurance forms
If you have dental insurance, you’ll need to complete the insurance verification form at your initial appointment. This allows our office staff to verify your benefits promptly.
You’ll receive a notice about our privacy practices regarding your personal health information. This describes how your medical information may be used and disclosed and explains how you can access this information. It also goes over the rights you have with respect to your Personal Health Information (PHI) and how you can exercise those rights.
Meeting our treatment coordinator
Once you’ve completed the new patient intake paperwork, you’ll be shown to a consultation room and introduced to our treatment coordinator, Angie. She’ll give you an idea of what you can expect from our practice, outline all the amenities we offer, and look over your medical and dental histories. Our records technician will then take a few digital photographs of your teeth. These are used to identify and discuss any orthodontic problems that are present. After the photographs are taken, it’s time for your exam!
Meeting Dr. Coleman
After being introduced to Dr. Coleman, he will perform a careful examination of your teeth and jaws. He may also ask some follow-up questions about your specific concerns as he is looking at your mouth. His findings will form part of your diagnostic records, which also include the digital photographs and any X-rays we take. If necessary, an impression of your teeth may be added to your diagnostic records as well.
Dr. Coleman will create a “problem list” using the data from your records. This is a customized list of everything that needs to be corrected during the treatment process, and it leads to a skeletal and dental diagnosis for you. This is a very important step in your orthodontic journey! An accurate diagnosis at this point will help ensure the most appropriate treatment plan for your smile.
The personalized treatment plan Dr. Coleman creates for you will contain each individual step of your orthodontic journey, from start to finish. It will include what appliances would be best for your case and an estimate of how long the treatment will take. If he feels that you’re ready to get started, we can often expedite treatment by taking records during that same visit. In some cases, we can even place braces that same day!
The investment required for your new smile
Once you’ve decided on a treatment plan with Dr. Coleman, we’ll go over the total investment needed for your new smile. Every case is different and each patient responds to treatment in their own way, so the total costs will vary from person to person. You never have to worry about hidden fees here at Coleman Orthodontics! We’ll make sure you understand everything that’s included in your treatment plan before moving on to the active phase of treatment.
The financial side of orthodontics can be confusing, but we believe everyone should have access to the care they need. Our treatment coordinator will review our financing programs with you. In an effort to keep orthodontic fees down while maintaining the highest level of professional care, we have established the following financial policy.
- If full payment is made at the onset of treatment, we offer a fee discount. If you have insurance, we will offer a discount for payment of your portion of the fee that insurance will not cover.
- To fit your individual needs, financial arrangements can be made to extend your payments over a period of months.
- We accept payment by Visa, MasterCard, American Express, and Discover for your convenience.
If you have dental insurance covering orthodontic treatment, we will review your coverage, benefits, and savings. Our team will be happy to assist you in filing your claim and handling insurance questions from our office on your behalf.
The official start to your orthodontic journey
The final step in your consultation will be completing a financial contract and treatment consent form to make everything official! Our financial contract is very straightforward. It outlines the total fees for your treatment, any upgrades included, and the expected insurance benefit amount. As the responsible party, this contract is your responsibility. In the unfortunate event that insurance stops payments, the remaining unpaid balance will need to be paid by you to settle the account. Be sure to review and clarify any final questions you have before signing your financial contract!
Your consent form permits us to treat you (or your minor child.) Legally, nothing can move forward without this form so take the time to examine it carefully. You’ll be expected to read and initial each statement on the form. This helps us verify that every point has been read and understood prior to beginning treatment.
There will also be office policies to review at this point. These generally include our policies on emergency visits, payments, and broken brackets or appliances. There will also be information on transferring or discontinuing your treatment.
